Below are downloadable Microsoft Word templates of the Agreement Appendix forms regularly used by faculty (in alphabetical order by name). Each can be typed into, saved, and printed but the text can't be modified. NOTE: the shaded areas are the "boxes" to type into, and they will expand on the page and beyond into a second page as needed.
